manage a large 70+ panel of Medicare patients with chronic conditions, delivering monthly clinical calls that drive real outcomes and reduce hospitalizations.<br><br>This role is built for nurses who can <strong>operate independently, manage complexity, and execute consistently at a high level.<br><br></strong>We expect our Care Coaches to treat this role as their <strong>primary professional focus</strong>, not a side and coordinate services</li><li>Update care plans and track interventions</li><li>Support transitions of care to reduce readmissions</li><li>Manage your panel efficiently and
